Triage X takes place in a city where organized crime, corruption, and street violence have pushed ordinary law enforcement past its limits. When the police can no longer reach the people driving that chaos, a secret vigilante unit called Black Label intervenes with lethal force. Operating behind the respectable front of Mochizuki General Hospital, the group is directed by hospital chairman Masamune Mochizuki. After a terrorist attack kills Arashi Mikami’s family and best friend, he is drawn into Black Label’s brutal war on the underworld and becomes one of its frontline fighters.
Alongside operatives such as Mikoto Kiba and Oriha Nashida, Arashi is sent after arms dealers, corrupt officials, gang leaders, and other dangerous figures who profit from the city’s collapse. Triage X mixes gunfights, close-quarters combat, dark humor, and an overtly sensual, fan-service-heavy tone, but its central conflict stays fixed on a team willing to answer every outrage with overwhelming force. As each mission pulls Black Label deeper into retaliation and shifting power struggles, Arashi and his allies keep fighting through a city that has long outgrown ordinary law enforcement.
