Chaos;Head is set in Shibuya, where the string of gruesome murders known as the "New Generation Madness" has made the district a place of fear, rumor, and constant speculation. At the center is Takumi Nishijou, a high school otaku who spends most of his time playing online games and watching anime, avoids the real world whenever he can, and slips into delusions so easily that he can never be sure whether what he sees is real.
Takumi's routine collapses when he receives a horrifying image from a user named Shogun: a man staked to a wall. After trying to calm himself at an internet cafe, he witnesses the same murder scene in person, and a pink-haired girl covered in blood appears before him, calling out his name. From that moment on, he is pulled into the New Gen murders and into a struggle to understand what is real, whom he can trust, and whether the killer is now targeting him.
As the incidents around Shibuya intensify, Takumi's fragile sense of reality is pushed further and further, forcing him to confront the boundary between perception, paranoia, and the violence closing in around him.
